Initialization code was split between kgsl_device_probe
and kgsl_register_device, which made the code unclear and
error handling difficult. Now all initialization of data
structures happens in probe, and kgsl_register_device only
manages the /dev file.

There are many fields in kgsl_device that are always initialized
the same way. Move them to a macro, KGSL_DEVICE_COMMON_INIT,
so the correct static initializers can be used by both 3d and 2d.

wwan serial driver sets suspend flag when it goes to suspend.
Driver puts any write request to delayed queue during suspend
and submits the delayed write requests while resuming and clears
the suspend flag after that. This creates a race condition where
write request comes between queuing the delayed write request and
clearing the suspend flag.

clk_prepare and clk_unprepare are now required to be called before
clk_enable and after clk_disable. Do this for the dma driver.
Additionally, split the msm_dmov_clk_toggle function into two, since we
always know the value of the second argument at compile time.

Converting the dma driver to mutexes is a requirement for using the new
clk_prepare and clk_unprepare apis. In order to deal with callers who
run in atomic context, refactor the enqueuing code to run in a
workqueue. Introduce a new spinlock that only protects the command lists
and not the hardware so the enqueue routine can add to the lists in
atomic context.

Previously there were two interfaces for flushing a channel, the
difference between them being rather ill defined. Both would end up
with the same result if used correctly, and they only varied in error
conditions.

If the A2 power collapse feature is disabled, bam_dmux will likely vote
for dfab twice in a row, which should not happen. This is because the
first vote is done when it is assumed that A2 PC will be enabled, and that
vote is not accounted for in the internal driver state when A2 PC is
disabled.

Metadata memory is not freed when there is
memory allocation failure in ddl open. Fixed
this issue by freeing the metadata memory.
